Sometimes you need to adjust a pre-existing form, adding new fields or removing no longer applicable ones. See our instructions below to add a new field to a pre-existing form. If you need to create an entirely new form, see the article Creating a Form.

Important: If you currently use or want to use a form in an order export, you will need to notify your customer support team. Form data is not automatically mapped into your exports. Any changes must be added by the development team via request through your support team, otherwise these fields will not appear within your custom export. |
Adding a field to a pre-existing form
- Inside of your Commerce account, click Store from the menu panel and select Forms from the drop-down menu.
- Click on the Form to which you would like to add or adjust the form fields. You can also click on the gear icon under the Actions column, then click Edit.
- To add a form field, click the Add a Form Field button.
- Choose the Type for what type of information will be collected or how this field will appear in the form.
- The Label will be the the name of the field, the Description can provide additional instructions for completing the field, and the Placeholder Text is what appears within the field (where applicable).
- The Subject Data Mapping setting is important when collecting pre-order data about the subject. See Understanding Subject Data mappings for additional information.
- If you would like this field to be mandatory for your customers to submit, make sure to mark the Required checkbox. Fields with an unchecked Required box will be optional for your customers to complete.
- Additional options will appear depending on the type of field added. Please contact your support team if you have any additional questions when adding a field.
- Add additional form fields for each piece of information you need to collect.
Note: First Name and Last Name should be separate fields, NOT combined, to avoid export issues.
- To remove a form field, simply click the gray "X" button next to the form field.
- To change the order of the form fields, click and drag the gray arrow button next to each applicable form field.
- Once your form updates are complete, click Save at the bottom right corner of the page.
